6.1 - AN EXPERIMENTAL COMPARATIVE STUDY OF ABSOLUTE DMSA-Tc99m RENAL UPTAKE IN RABBITS.

The absolute quantitation of DMSA-Tc99m renal uptake permits better evaluation in bilateral renal disease and in patients with a single kidney. The purpose of this study was to investigate whether or not a different formulation of radiopharmaceutical DMSA and the relative concentration of reagents, would have any influency on degree of in comparison to a standard product used in the market (CIS bio international). Methods: 15.86MBq/Kg of DMSATc99m was injected into an ear vein of New Zealand rabbits. Images were obtained, 6 hours later and the calculation of absolute renal uptake was done, expressed in percentage of the administered dose. Twenty eight animals were studied using each formulation. The rates showed no statisitically significant difference (p=0.01 and p=0.05) in comparison with the TCK-12. Considering the limitations of this study the conclusion reached was that different formulations did not change the renal uptake in rabbits 6 hours after injection.

6.2 - RADIONUCLIDE SCAN TO RENAL TRANSPLANT

Complicacions of transplanted kidney was investigated with radionuclide scan and compared with histopathologic and others immaging procedures in order to define the scan sensitivity. Retrospective analysis of patients records was done from 1993 to 1998 period. Five hundred twenty one exams from 168 patients were reviewed , all of them done with 99mTc-DTPA. The scan analysis was done visually and quantitativally to the perfusion phase and 61,3% of patients demonstrated scintigraphic alterations, as follows: ATN - 80,6% ; Acute Rejection - 29,1% ; CyA Nephrotoxicity -10,6% ; Renal Infarct -5,8% ; Urinary Leak -1,9% ; Lymphocele -1,9% ; Vascular Trombose -1,9% ; Renal Hematoma -1,9% ; Hidronefrosis -1,9% ; Intrarenal Pseudoaneurisma -0,9%. The overall sensitivity was 96%.

6.3- STANDARDIZATION OF THE METHOD TO CALCULATE THE ABSOLUTE RENAL UPTAKE OF DMSA - TC99M IN CHILDREN.

Our purpose was to evaluate and to establish the normal values of absolute renal uptake of DMSA -Tc99m in children with preserved renal function , as correlated with the clearance of creatinine. Twenty two children (aged 7 months to 10,4 years, mean 4,5 years) without clinical evidence of renal pathology were studied by DMSA-Tc99m scintigraphy. Eighteen had normal renal ultrasonography, micturating urethrocystography, creatinine clearance and visual assessment of the DMSA-Tc99m scintigraphy. Four children were excluded, one because didn't complete the creatinine clearance and three because they had reduction of the creatinine clearance, with other normal parameters. The relative and the absolute renal uptake of each kidney were calculated related to the administered dose. The absolute renal uptake of the DMSA-Tc99m was 22 ±3% for the right kidney and 23 ±3% for the left kidney. The values of the absolute uptake showed little variation in normal patients in different ages, distinctively to the observed with the creatinine clearance, which exhibited physiologic variations in relation to the age of the child. The study suggests that this method can be employed in the diagnostic evaluation of renal pathologies, specially in bilateral attempt or single kidney.

6.4 - RENOGRAPHIC PATTERN CHANGES IN A PATIENT WITH MOBILE KIDNEY: A CASE REPORT

The authors present a case of a white female patient, 36 years old., who developed arterial hypertension 5 years earlier, during pregnancy. She also had a history of urinary tract infections, renal colics and lithiasis. Radiologic evaluations found, on ultrasonography and urography, right renal ptosis. Renal scintigraphy with Tc-99m-DTPA showed normal blood flow for both kidneys while patient in supine position. The right kidney showed mild caudal shift and glomerular function mildly reduced. The left kidney showed normal function. The study was repeated with patient in upright position, so that was noted an increased caudal shift of right kidney, with significant reduction on its blood supply and glomerular function. These finds suggest that transient renal abnormal position can produce remarkable changes in renal functions (reflected in renographic curves) which is related to arterial hypertension.
