29. SERUM ß2-MICROGLOBULIN AS MARKER OF GFR

AIM: To compare serum creatinine (Cr) and serum 2-microglobulin (ß2M) as marker of GFR.

PATIENTS: 178 adult pts with various chronic kidney diseases, 88 M and 90 F, aged 16-83 yrs (mean 55), body weight 43-108 kg (mean 68.5), with normal muscle mass and nutritional status, participated in this study. Their renal function ranged from normality to advanced renal failure (Cr 0.40-13.30 mg/dl), mean 1.77).

METHODS: Cr has been measured by autoanalyzer,ß2M by RIA and GFR by the bladder cumulative method using Tc99m-DTPA as glomerular tracer. GFR has been adjusted to a body surface area of 1.73 m2.

RESULTS: The results are reported in Tables I and II

In pts with impaired renal function ß2M increases and more than Cr.


A linear relationship was observed between ß2M and Cr (y= 2.170 x+0.317, r=0.890, p<0.001) as well as between GFR and reciprocals of Cr (y= 0.009x+0.304, r=0.792, p<0.001) and ß2M (y=0.005x+0.089, r=0.773, p<0.001).

CONCLUSIONS: These results indicate that ß2M is a reliable marker of GFR, alternative or complementary to Cr. Its clinical interest is higher in patients with minor degrees of renal impairment .
