28. LONG-TERM RESULTS OF OPEN PYELOPLASTY EVALUATED BY PROSPECTIVE DIURESIS RENOGRAPHY.

OBJECTIVES: To examine the durability of the Anderson Hynes pyeloplasty over time by the prospective use of MAG-3 diuresis renography 9-19 years after surgery.

PATIENTS AND METHODS: the records of 56 cases of Anderson-Hynes pyeloplasty performed by 2 surgeons between 1981 and 1994 were examined. Attempts were made to trace all patients, and to perform a current F+20 MAG-3 diuresis renogram to examine the status of the operated renal unit.

RESULTS: 41 evaluable cases were identified, 24 patients agreed to return for reassessment. The mean time from surgery was 10.6 years (range 6-19 years). The results demonstrated improvement over pre-operative split renal function in 19 cases (79%), and improvement in drainage in 23 cases (96%).

CONCLUSIONS: Most of the newer endoscopic procedures for UPJ obstruction have never been validated by pre-post-and distant use of renography. These results demonstrate that the Anderson Hynes Pyeloplasty is an excellent open procedure for the treatment of UPJ obstruction, and produces a lasting improvement in function and drainage in the majority of cases. It can truly be said to be the gold standard against which newer techniques should be compared. Validation of new techniques by the use of renography should be encouraged.
